The Bosch Visa Prepaid Card can be used at millions of locations worldwide, wherever Visa debit cards are accepted. Because it’s a

prepaid card, spending is limited to the amount of money placed on the card. Use the Card to buy what you want, when you want. Like

other Visa cards, the Bosch Visa Prepaid Card offers all the protection and security features you’ve come to expect from Visa. So unlike

cash, the Visa prepaid card can be replaced if lost or stolen.

Disclosure: Cards are issued by Citibank, N.A. pursuant to a license from Visa U.S.A.

Inc. and managed by Ecount, a Citi company. Cards will not have cash access and

can be used everywhere Visa debit cards are accepted. Using your Bosch Visa Prepaid Card

for a purchase is similar to using a credit card. Know your balance before you shop, as

not all merchants can determine it for you. Each time you use your Card, the purchase

amount is automatically deducted from your card balance. Cannot be combined with any

other offers.

Instructions:

1. Go to http://boschappliances.fasterreward.com to register or

complete ALL Information on this form including FD number

for the product. Incomplete forms will not be paid.

2. Include purchase invoice or sales slip (circle the model number

and date of purchase) and this reward form.

3. Mail no later than March 15, 2010, to:

If you do not have your FD number because your

appliance will not be delivered by the postmark date:

1. Mail this form (without FD number) as instructed in steps 1

through 3, by March 15, 2010.

2. Call 1-877-582-4040 within 45 days after the postmark date to

submit your FD number. A reward will not be issued without a

FD number.

(collectively “Claim”) must be mailed and postmarked by March 15, 2010. No late

3. Make a copy of all reward material submitted for your records. In the event of

incomplete or illegible forms, claims submission will not be returned. 4. Offer good only on

occur between February 1, 2010, and February 28, 2010; purchase outside

these parameters is ineligible. Purchase date is determined by invoice. 6. Any

misrepresentation or fraudulent information disqualies reward claim, and may give rise

to criminal or civil prosecution. 7. Offer is not valid in conjunction with any other offers.

8. Sponsor(s) distributors, resellers and their employees, are ineligible. 9. Valid

only for purchases by an end-user customer within the 50 United States and the

District of Columbia, Puerto Rico and U.S. Virgin Islands. 18 years or older only.

10. Reward form must include a valid, current, residential street address of

the purchaser only. Visa prepaid card is not transferable. 11. If these terms and

conditions are not met, at Sponsor’s discretion, Visa prepaid card will not be issued. Offer void

be liable for more than the amount of the stated reward; by submitting this reward form,
